Tennessee prison chief dismissed after failed execution
The execution of Krista Pike in the US state of Tennessee is considered unsuccessful, as she survived the lethal injection after two doses of the drugs led to her losing consciousness. This incident resulted in the resignation of the state's Department of Corrections director, Frank Strada, and the suspension of all executions in Tennessee. The event sparked a debate about the flaws in the death penalty system, with Tennessee's governor affirming that the corrections administration failed to properly follow protocols and that the execution attempt revealed deeper structural deficiencies within the system. Pike had been sentenced to death for murdering Colin Slymer during her teenage years, but complications arising after the failed execution have intensified criticisms regarding the effectiveness and human rights concerns related to the incarceration system.
